Use this form to send a clear withdrawal statement for an eligible distance service contract. Submitting the form does not require you to give a reason.
If you are exercising a statutory right of withdrawal, submit the request within the applicable period. For EU/EEA consumer service contracts this is generally 14 days after the contract is concluded, subject to applicable exceptions and rules for services that have already begun.
